I can't say since which of the last updates this occures, 1.0.6.285 or 1.0.6.284, but my actual installation (1.0.6.285) is a complete nightmare.
It began with a complete loss of all playlists (music was still present).
Then I synced with actual MMW via WIFI and every time I do that, a new profile is generated.
It was not possible to get my playlists back by syncing, so a changed the playlists-folder and it helped -> not nice, but working.
Then I synced some new songs and the syncing rule for all songs (/Music/<folder name>) was ignored. I now got something like /Music/<Artists> for all files.
I then decided to uninstall MMA, delete all files and start over again.
Now most files seem to get synced to the correct folders but a lot of files get synced to /Music/ like this.
expected: /Music/Electro/someSong.mp3
found: /Music/Electro_1 -> with rising numbers.
Furthermore ratings don't get synced to MMA.
All in all... Frustrating!
I'm using Samsung Galaxy S4, Sandisk 128GB microSD, Android 4.4.2, MMA1.0.6.285 ; Windows 8.1, MMW 4.1.3.1707

As far as Paths on device go, did you correctly set them up in the Device Profile that's being used for the sync?

Another example:
there's a folder "Music\Electro\Avicii" with 3 subfolders and about 20 songs inside on MMW to be synced.
After sync I find on MMA:
Music\Electro"many Avicii songs with random doubles that should be in folder Avicii"

"Music\Electro\Avicii\"3 subfolders with correct content"
+
"Music\Electro\Avicii\many songs with random doubles"

As you have multiple Device Profiles for the device it wouldn't be surprising if some were used with incorrect Path settings.
Are you allowing MMW to delete files not on the sync list (in the Device Profile)? Note this if files were removed from the MMW database (even if they were then added again) that the option needs to be set to all media files.
