Moved a bunch of files at once, and one has disappeared

Just moved a group of about 100 files using the context menu "Auto-Organize," into about 10 different folders. All of the files moved properly.
One file moved physically, but not in the database, and is still pointing to the old location.
Database was given a full Optimize about 5 hours ago. No file handles to the drive except Media Monkey. No files in Now Playing were moved.
No debug log, because I can't reliably re-create this. But it has happened before when I move large groups of files.
