4.0.0.1344 iPhone sync still removes everthing first

This has been reported in a couple of other threads but I haven't seen an actual bug filed for this.
In MM4 (still happens in 4.0.0.1344), when I sync to my iPhone 3GS, MM always says that every track and playlist already on the phone isn't on the sync list and wants to delete them all prior to syncing. If you do this, everything syncs OK, but it obviously takes a very long time. This never happened in MM3.
I am syncing three auto playlists:
1. "The Sync List" - Playlist of all tracks rated 3 stars and above - Sort order unspecified
2. "Excellent Tracks" - Playlist of all tracks rated 3.5 stars and above - Sort order randomised
3. "Absolute Favourites" - Playlist of all tracks rated 4 stars and above - Sort order randomised
At first I thought it was down to either the randomised sort order or the fact that playlist 3 is a subset of playlist 2 which is itself a subset of playlist 1, but it seems not.
I then changed the auto sync to just sync a single album. As expected, it prompted to delete the rest of the content which it appeared to do, then sync the selected album. However, on checking the device, nothing had actually been deleted at all!
I then removed all content from the device and verified it had gone (MM4 was showing the storage as being free as expected) and again synced the single album. After completion, I then synced again, and once again MM wanted to delete the existing content, claiming it was not in the sync list.
Something does seem to be seriously broken with the iPhone sync function that was not a problem in MM3.
