Was Playing selected in the Media Tree or an AutoPlaylist?

If Playing (but it has no Grid option so probably not) you need to enable the option to remove the played files.

Can you please share your database file (mm5.db) with Support (please compress it (ZIP))?

If the debug build runs without issue there is nothing to be done. You can try installing the normal build to see if the issue returns, or keep the debug version running.

Did you scan files anytime before you Locate Moved/Missing Files? That would explain the duplicates. Using Added information you can remove the recently added, leaving the original files.

What Build (Help > About) of MediaMonkey are you using and did you do a regular or portable install?

This is not possible, but you can do an iTunes XML export if the software you use supports it.
