Yes, that has happened to me too.
And I also have 'Scan file directories for Artwork' disabled.
To be safe, I now store my artwork in a sub-folder away from my music files.

I save pictures album exclusively in the TAG.
The Option 'Scan file directories for Artwork' is disabled.
Recently (probably since version 1408) MM adds all the images located in added folders to the library.
Now I have to manually delete these Artworks from the library.
