Strange, because AFAIK the [+] creates the exact copy of current tab. I cannot replicate and also don't see a reason for it.
Maybe there are some specific steps to reproduce, but I haven't figured them out.
No, it's hard to reproduce intentionally. Happens to me once and a while, but: It just happened! Freshly started MM, navigated in the left pane to one of my music directories, pressed '+' and got a new tab with all the correct columns but with incorrect column widths.
I took screenshots, however they just show what I described...
