I'm having a similar issue to this with my Nano 6G. Some weeks ago I wiped the iPod using someone's iTunes, then re-started the sync list. I play some songs; I connect and scan the info; I disconnect. I haven't changed the list since making it.
I just connected the iPod after some plays over the past couple days, let it scan, and then sync'd. I watched the status bar to see what was "copied". Some of the tracks had recently been updated on the computer; others haven't been played, or edited, or given new artwork: not touched or heard at all. I think the "copies" were all actually updates; they seemed to go faster than adding new tracks. (It would be nice if the status bar could show "Updating" instead of "Copying" when that's what's happening.) Is there a sync log available?
I tried to test whether last.fm playcount importer could have affected this, but I got a crash when I tried to sync after running the importer. I had the debugger version running, but the error was not caught and kicked back to Windows: "MediaMonkey has stopped running" with a Close Program option.
