That's the only one I see. I organize my toolbars close MM and when I restart they're back to original. However what I noticed that this is additional toolbar that was created when I installed some scripts (Genre Finder/Batch Art Finder). The original toolbars from MM are fine.

I try to explain what happens a bit more on my sytem. It's a fresh and clean MM 1367 install on Win7 x64 SP1.
1.) Whenever I click OK in Toolbar Configuration Menu all custom toolbars (including newly added ones) disappear. To be shown, they have to be ticked in context menu again.
2.) A first custom toolbar named "1" is added and placed to the right of the original "context" Toolbar. ...fine
3.) A second custom toolbar named "2" is added. It takes the place of "1" to the right of the original "context" Toolbar. "1" automatically moves to the left of "context" toolbar.
4.) A third custom toolbar named "3" is added. It finds its place to the very left, moving all others a bit to the right. ...fine but "1" is automatically renamed to "2" so I get twice a "2"
5) creating a fourth woul rename the initial "1" to "3" and so on. Opening the configuration Menu a few times leads to several toolbars named "4" and one named "3" without having renamed them manually.
6.) Some of the bars would loose their position after having moved them manually. some wouldn't. Coldn't figure out how that worked.

My steps
1) Create toolbar named 1
2) OK
3) No toolbar 1 exists as no icon was added

Take away
1) New toolbars should be enabled by default, edited toolbars should maintain state enabled/disabled state from before editing

Reported issues are working correctly for me in 1370 now.
But there's a new one:
If you have only one single row of toolbars, you cannot place the icons above Mediatree and Now Playing. Disabling them via "View" makes it possible to place toolbars where you want. Enabling Now Playing and Mediatree afterwards lets the Icons stay there.

I can't reproduce that, I have all toolbars on single row and can place them in the Media Tree or Now Playing.

I can place them in Media tree and Now Playing as well. Then it ads sort of a short second row with the width of the Playlist. But not in the single row above MT or NP.

OK, this is about the furthest I can drag the toolbar to the left.

If I go further it either jumps to the top into the row with Menu and Tabs...

...or it jumps a down and creates this second row with the width of media tree.

Only if I disable MediaTree in "View" Menu I can pull the toolbar to the very left. Enabling Media Tree again leaves the Toolbar in its place.
