I appreciate your help, for sure, but the user shouldn't have to worry about using additional features for something that should / could / was been done during Add/Rescan. If not, that feature should be called Add/UpdateTag, per definition not Rescan
In the version MediaMonkey_4.0.6.1501 I'm not prompted for the files concerned ! There is a message of total number of files; number added, number updated.
I have bad feelings about the MM design evolution since MM4.

Try the following:
1) Copy some files into a folder
2) Scan that folder in
3) Remove some files using Windows Explorer from that folder
4) Rescan the folder
Making sure that the option to remove unavailable items is checked does it prompt on the removed files?

I'm not sure what can cause this nor can I reproduce so it's best you create a debug log (step 4b) and send it to support: http://www.mediamonkey.com/forum/viewtopic.php?f=1&t=69 (you can use the steps I outlined above).
