Thanks for the report!

We'll look into the log, but in the meantime can you confirm:
- Does the issue occur consistently for certain tracks or is it truly random? If so, are there any particular characteristics for those tracks?
- Do you have Options > Playback > Playback engine set to 'Exo'?
- Have you enabled Options > Developer options > Level volume:Exo? (this fixes https://www.ventismedia.com/mantis/view.php?id=11957 and we didn't include it as a regular feature because we didn't have time to fully test it)

New problem with current release with gapless on or off. Sometimes, a track will start playing back loud and then reduce to a normal level. Random issue.
