I have just uploaded an updated version that fixes problems of album art not appearing at all on iPhone 3GS and some minor tweaks.
And a clarification regarding known problems of the current version of the plug-in on 3.x firmware:
1. It doesn't cooperate well with iTunes - previous content of iPhone is often ignored. We are looking into it.
2. In some cases the name of the device disappears. You can edit it in MM tree and it should be fine then. We are looking into it though.
3. Album art problems were hopefully fixed, there aren't any known. Some of the reported look like that old, sometimes appearing issue of incorrect album art that however haven't been reproduced yet and it isn't clear whether it really exists and what's its source.
4. Synchronization fails completely. We can't reproduce, in case you have such a problem, please create a debug log and e-mail it to me.
There aren't any other know problems.

Completely restored my phone last night an MM reports 32gb correctly now. I can also confirm that album artwork is indeed showing up on my phone with todays updated dll. Good Job!

the issue w/ Album Art not showing up. Do I need to restore using iTunes, or I can just delete the contents
via MM and resync via MM?

This is unrelated but I have a question is there a way to sync ringtones. I have alot of ringtones I made I used to sync to my Motorola phone. Itunes of course wont let me use them b/c they are not from songs I paid for.
So do we have a way to sync them to the ringtones folder of the iphone?

I used this to push ringtones into itunes:

I was able to get what I needed into itunes free...
As I explained in above post...I can then sync m4r files to iphone with itunes...after that you can use MM for syncing....JUST NEVER delete the m4r files from the iphone in MM.
Hopefully MM will make accomodations for ringtones soon.

This worked great but now I noticed a new issue with Syncing with MM
Anything I synci with itunes (podcasts or ringtones) are removed from the phone when I do the next sync with MM

Hmmm.
I have synced several times with all the new dll's this week and all my ringtones are still there.
I have the ringtones located with all my music files on NAS...I don't let itunes decide where to put them. Not sure if that might be an issue. On the iphone itself they are located at:
\iTunes_Control\Ringtones\xxxx.m4r
You know, with the goofy four letter "code" name.

Well it looks like its itunes doing something, I F'ing done with them in trying to get my ring tones on it fucked my whole lib up so I had to remove all with MM now I am restoring.
The tracks were shown just would not play.
Ive lived with out ringtones for this long guess I will just have to find a way to load them with out itunes

The album art was ok till iTunes analyzed the music for uninterruptible playback. After that all album art was gone.

Synced with the newest version of the DLL yesterday. Just about all the issues I have with my 3GS running 3.0.1 are gone.
One small problem remains. MM scrambles some of the artwork and puts the wrong cover art with the wrong album or it will give me the upper half of one cover with the lower half of another. Don't know if this is a unique problem to the new DLL or has been an issue with Album art in the past with other models.
Any suggestions?

The scrambled art seems to be an ongoing problem with Apple devices and MM.
It happened with me with original series of Nanos a while back and there seem to be many examples on many different Apple devices.
My problem seemed to be eventually resolved by finding specific track files with embedded art that were causing issues.
Sorry about all the "seems" and "seemed"...but that SEEMS to be the nature of the issue.
All my art works now...but as has been pointed out- iphone seems to be very sensitive concerning artwork...stuff that might pose no visible issue in MM or other players seem to cause problems with Apple devices/software.
Clear and restore device...slowly load tracks till problem occurs and then reload that artwork or simply replace album art of everything...this is what I did and things are working here. Once art works properly on Apple devices the problems haven't reoccured.

Remember... if you update ringtones in itunes ANY MUSIC previously loaded by MM will be lost on the device. The ringtones seem to be connected to the music file structure in the devices. (I'm not sure about video or podcasts because I don't have any...might be same issue...experiment)

I'm in the same boat with the same setup - the latest dll now loads the artwork, but some are scrambled. no ringtones.
Other than that, its working well. I may be having an issue with play counts not getting updated back from the iphone.

Abbraxxa, I loaded my whole whole library into itunes and everything was once again "analyzed for gapless playback" by itunes just days before the new dll came out. The art that was in associated files still worked on the 3g here.
