I don't know, I have an iPhone and iPod mini that don't exhibit this problem. Maybe it is your filename destination, I see you use:
<Artist:12>-<Album:12>\<Track#>
Because of the way the iPod filesystem works, all filenames can only be 4 characters long (i.e. Thre.mp3). Maybe you using <Artist:12> is forcing the filenames to not be written correctly. In the past I've used masks: <Artist> - <Title> and: <Title> with success, maybe it's worth trying. The only downside is that you'll probably be prompted to delete everythinga and resync everything.
