I am still having many bugs with MM and playlists on my 4th generation iPod touch (IOS 5.1.1).
There are 4 playlists on my iPod. When I just connected MM scanned and now has the following:
4 copies of Jazz 1000. All are identical, but each has only 78 tracks listed; Jazz 1000 actually has 552 tracks.
2 copies of All Jazz. Both are identical. MM says there are 797 tracks; there are actually 2222.
1 copy of Jazz Classics. Here MM gets it right reporting 671 tracks, which is what it has.
3 copies of New Jazz. All are identical with 128 tracks. There are 271.
This is what seems to me to be happening. I originally created all these playlists in MM and synced the playlist to the iPod. The number of tracks MM reports for each playlist are the number that were originally synced. Since then I have added additional tracks to the iPod and to three of the playlists. The number of extra playlists MM reports seems to be the number of changes I have made to the playlist on the iPod. So it notices that changes have been made, but doesn't actually scan the iPod for the content of the playlists.

Because I have auto-sync turned off (I really don't like letting programs alter what's on my iPod without my knowing what it is doing - didn't let iTunes do it, either), I thought maybe just the initial scanning wasn't enough so I did a sync, with these results:
MM created a second Jazz 1000 playlist on my iPod with 997 tracks
MM reports 4 copies of Jazz 1000, each now with 160 tracks (should have 552)
MM still has 2 copies of All Jazz, but each now has 1930 tracks (should have 2222)
MM still has 3 copies of New Jazz now with 135 tracks (should be 271)
And now some more data.
The playlist All Jazz includes Anat Cohen's Clarinetwork. The album has 8 tracks. All 8 are on the iPod and when I look at the playlist All Jazz on the iPod itself all 8 are listed in the playlist. But when I look at the iPod playlist All Jazz in MM, only 5 of the tracks are listed. If I look at the music that is on the iPod using MM, all 8 tracks are listed. That is, MM finds all 8 tracks but says only 5 of them are on the playlist. And here's the kicker. This album got put on the playlist twice, though the two entries are a number of albums apart. MM makes the same mistake on both entries. It lists both, but misses exactly the same three tracks both times. There is a copy of the playlist All Jazz on MM itself. It lists all 8 tracks of this album. Why is MM not finding some tracks in an iPod playlist (and why does it miss the same tracks twice)?

I cannot replicate this. Could you find some consistent steps to reproduce?
I have tried this:
1. Right-clicked playlist called "Sync" and selected "Send to->iPod Touch "
2. Verified that after the sync the playlist is there
3. Modified the playlist in the device
4. Again right-clicked playlist called "Sync" and selected "Send to->iPod Touch "
=> playlist is in the device once, the only issue is that changes made in the device were overwritten by re-syncing of the same playlist

You last comment is why I don't like to sync MM playlists to the iPod a second time. I do make changes in the playlist on the iPod and I am not always at my computer. I would find it helpful if you could explain exactly how playlist syncing is supposed to work. MM help has nothing to say about this.

Note that PM shown that the important steps to reproduce this is to create the same named playlist beforehand directly in the device and then sync the same named playlist to the device. The result is that in the device are two playlists with same name and including content of both the playlists (created by MM and created in the device)
