In 2015, MM still has this bug. The solution is to run MM as Administrator and then Tools > Advanced Tag Management > Synchronize Tags command (shortcut: CTRL + S). Not only will it update file tags correctly, the speed of tag updates will be far faster than it normally is.

This bug is most definitely fixed, though it's possible that you're experiencing a similar problem.
My expectation is that since running MMW as admin solved the problem, that the issue in your case is a rights issue (i.e. when MMW isn't run as admin, it wasn't able to modify the tags of certain files because it didn't have rights to do so).
