Ok. From what I understand, given everything I've tried and resources linked here, MM4 creates a record in the Covers table but it doesn't store the blob itself.
So what it must be doing is that if a track has a Covers entry it will look up the artwork in the file tags.

If that is the case, then if, for some reason, the artwork is removed from the files without MM knowing, it will keep an innacurate record from then on.
Which also would mean that the only way of finding files in these conditions would be through some external tools to find files without this tag. Or by using a portable version to scan everything from the start.

What I don't get is why rescanning the library doesn't update these files. This seems to be the problem, I think.

Hi, thanks. Although I think I already figured it out. Your case is a bit different from mine because you have artwork in the folder and I only use tags, but it's basically the same issue.
The only problem is that when MM rescans the files, it doesn't delete the lines in Covers for tracks that had artwork but now it's missing. It's a bug and there's not much I can do about it, except installing a portable version now and then and scanning the library fresh. Not something I look forward to, as each new scan requires several hours.

Anyway, thanks for the help. I did figure it out eventually, although it didn't help me much with what I needed.
