I'm going to analyze it a bit further before I send it in. It's so infrequent, that I'm going to try and keep of log of what other programs I have open when it happens, etc.

Look under Tools / Library / Appearance. It looks as though that may help you.

It was the File Monitor. It was set to the physical folder instead of the network location. Or, at least, that seems to have fixed it. I've edited a few tags and so far, no dupes.
